Based on the Intake Assessment, upon entry into your program and every six months thereafter, a Plan will be completed to identify socialization, emotional, mental, physical, and independent living skills needs.  A plan is formulated to address meeting each individual need identified.  

Persons responsible for the implementation as well as a method of evaluating progress are identified.  The client, therapist, and administrator sign off on this working document demonstrating a mutual awareness and commitment to working toward achieving the goals.
